Evolent Health and Community Care Cooperative to Transform Value-Based Care in Massachusetts
WASHINGTON, May 9, 2017 /PRNewswire/ -- Evolent Health (NYSE: EVH), a company providing an integrated value-based care platform to the nation's leading health systems and physician organizations, today announced its partnership with Community Care Cooperative (C3). C3 is currently serving as a pilot Accountable Care Organization (ACO) comprised of Federally Qualified Health Centers (FQHCs) and serving communities across Massachusetts. The partnership will bring together the best practices from both organizations to promote high-quality, cost-effective care for adults and children enrolled in the state's MassHealth Medicaid program and assigned to the C3 ACO.
Evolent will bring to the partnership its proven predictive analytics, technology infrastructure and clinical models implemented across the country, while C3 will bring years of market-leading experience delivering integrated physical and behavioral health in diverse local communities. Together, the organizations aim to successfully build a value-based care delivery system that could serve a significant number of MassHealth beneficiaries.

The Evolent-C3 partnership will initially focus on building a model of care and associated systems and processes that integrate social determinants of health and behavioral health services with primary care to optimize health outcomes. The companies will also deploy a common technology platform across C3 health centers with a goal of unifying data aggregation, clinical stratification, and coding and documentation workflows across C3's FQHCs, which leverage a variety of electronic medical record platforms and instances.
The Commonwealth of Massachusetts has long been a leader in health care, passing universal coverage in 2006. It is once again a model for innovative care by enabling organizations like C3 to serve beneficiaries in a Medicaid ACO. In November 2016, CMS and the Commonwealth announced the renewal of the Commonwealth's 1115 waiver, paving the way for work to restructure Medicaid delivery to support the move to ACOs and allow for innovative ways of addressing the social determinants of health. Massachusetts is the first and only state embedding social determinants of health into risk scores, ensuring that health-related social needs are recognized for the true impact that they have on an individual's cost of care.

According to a November 2016 study in the American Journal of Public Health, FQHCs provided a more cost-efficient setting for primary care for Medicaid patients. Those who received the majority of their primary care from FQHCs had lower use and spending than non-health center patients across all services, including 22 percent fewer visits and 24 percent lower total spending.
