Citrus Valley Health Partners & USC Collaborate for Innovation Population Health Conference
COVINA, Calif., Oct. 16, 2018 /PRNewswire-PRWeb/ -- Citrus Valley Health Partners and Keck School of Medicine of USC are proud to join together to create an exciting and inclusive forum on the health care ecosystem of the San Gabriel Valley.
With the theme "Medical Innovations," the 2018 Population Health Conference is designed to foster evidence-based dialogues and actionable solutions aimed at transforming the way health care is provided in the region. Key to our approach is the inclusion of innovators that provide tech-enabled solutions using social determinants to drive efficiencies, access to care and precision medicine to pin-point better health outcomes for highly-targeted communities.
Keynote speakers are Dr. Este Geraghty, Chief Medical Officer & Health Solutions Director of Esri and Sameer Badlani, MD, FACP, Chief Health Information Officer & System Vice-President for Sutter Health. Breakout session topics include: Mobile Health Technology & Diabetes Risk Reduction, Applications of Behavioral and Health Economics to Value Based Care and Innovations in Valvular Heart Disease.
